This form is provided for customers who have purchased the Persyst EEG Suite.

You will need the following items to register your software:

• Your Persyst Software Serial Number
• The registration file created by RegCenter (for instructions on creating a registration file, see below)

To create a Registration File:

1. Install the software.
2. After completing installation, go to Start | All Programs | Insight | RegCenter.
3. Select "Save to File".
4. Fill in all of the information. In the field marked "Serial Number" insert your Persyst Software Serial Number (which should be on your CD or on the back of your CD case).
5. Press "OK" and you will be prompted to save the file. Save this file in a location where you can find it, or to a removable media that you can take to a computer with Internet access.
6. Upload the file using the link above.
